Soft Tissue nose surgery cannot compare to moving the bone to make more room in your nose. This was a helpful quote for me as I'm trying to decide between an expander (again, had one around age 12) or septoplasty.
Extremely excellent video on this subject. You went so far as to describe DOME as well. I would only recommend if you were to update this video or modify anything, is to include screenshots of each of these treatment modalities that you mention. I like the fact you used the appropriate model as well. Just note that you mentioned splitting the skull* when you're just splitting the jaw* toward the end. Just a minor taxonomy correction. This gave me a better understanding of these processes so when I do a follow-up with my own doctors, I know what questions to ask. I may reach out in the future about a project I'm working on related to all of this :)
Something else I would say Ronald, is that you talk a lot about nasal volume being restricted by the size of the cavity. Another cause of airway restriction is a weak tongue which is low in the mouth - encroaching the airway. You can improve your airway through myofunctional therapy and shifting the mandible forward. As well. Great channel!

Much of the benefit of MSE can be obtained naturally through self-discipline and time -- _without_ all the risk. i am personally looking into MSE because i want to reopen my recently extracted wisdom tooth sockets and place implants there to stave off the bone-loss, and secondary to that i have UARS, so i believe the procedure is an unusual compliment for me... for most ppl i would rather think it beneficial to take your advice

I had MSE done as part of my brace procedure for my open bite. It didn't seem to expand my maxilla that much considering there was no teeth gap happening and no change in my facial appearance. I'm happy with the results though as I have no open bite anymore
I have both problems of cross bite and open bite together.... For Cross bite correction, i have to expand upper jaw..... Today my dentist wear me expander to my upper jaw...... Pray for me... How long your treatment time?? your open bite gap was _?__nm...?
This model isnt right tho because it acts as if the cheekbones are only connected to the maxilla, when in reality they are connected to the other long bone that goes horizontally along each side of your skull. And those bones then connect to other bones. So how would the cheekbones move without every single bone in your skull moving?
My question with this would be what happens with the septum? Does it stay perfectly in the middle? What if it swings to one side of the split? - that would be a huge issue
10 mm of expansion is by no means a world record. Dr. Mew regularly expands his patients by 10-15 mm, and he said during a webinar that his greatest expansion he ever achieved was 24 mm. Not MSE though obviously
@@disillusioned2710 I consulted many orthdontist and non of them recommend anny kind of appliance to adults, it's a tricky situation if you go and pay allot of money for something and your suture doesnt split and appliances like Bioblock , DNA and so on only make dental movements. They all recommend SARPE for definitive spliting of the suture and widening of the palate. all other stuff is minimal results with big payments or just aimed for people who dont have hardened sutures yet, younger people.

Since it's bone borne it won't cause any harm unless your ortho messes up big time. Maybe it fails, but you can try again, SARPE in my opinion is stupid if you r looking for any kind of aesthetic benefits... If you are just looking to get a proper occlusion, you can go for sarpe but if you can get mse, it's the best imo.
I hope you can answer this as I have seen it being talked about many times in your comment section, can you do anything about the Interpupillary Distance? Does MSE do that at a young enough age? Do you have to assist this with puncturing nasal sutures?
what about facial aesthetics? Wouldn't this also widen the nose and decrease upper lip tissue volume, both side effects that more often than not negatively effect most peoples appearance?
I started Myofunctional therapy and told them I’m considering mse. And they said my intermolar width is 44mm so I don’t need it. But it really doesn’t feel that wide.
In the “dome” procedure you claim it’s done to guarantee that the mid palate suture separates. But a draw back is that it doesn’t allow for mid face expansion. What is the reason for cutting the zygoma, if you’re really just after “assuring the palatal suture gets split? Seems like a better, and less invasive, procedure would be to just do 1/3 of the “dome procedure”... just surgically split the palatal suture and leave the zygoma bones alone.
Super late reply, but let me clear this up. The main benefit of DOME isn't to overcome the resistance of the mid-palated suture, It's to overcome the structural resistances of the skull itself. And you don't cut this zygoma, you cut the maxilla with a cut called LeFort 1.
So, you expanded the maxilla by 1 cm. What about the centimeter cavity that appeared after that? Do you now just have a large cavity / crack in the middle of the upper jaw ( like on 1:10 )? Does it feel? Does it interfere? Or is it overgrown with cartilage / a new bone has formed in this place?
In 6-12 months the split will fill with boney material. The MSE has to stay in until this happens. That's why my MSE is still in. This is similar double jaw surgery where a large section of the maxilla is pretty much cut off of the face and repositioned in place with metal plates, and then heals into a solid, contiguous piece of bone in 6-12 months.
Unfortunately no one knows exactly what the AGGA mechanism is. It's some combination of tooth pushing, tooth tipping, alveolar bone pushing, and remodeling. Mostly the first 3. One thing is for sure: don't believe a word you hear from the AGGA providers.

I had braces in my early teenage years. They removed 4 adult teeth which screwed my jaws up, made me a horrible mouth breather, with chronic sinus issues. Now I am trying to determine whether I want to do MSE or EASE. And possibly add back the 4 adult teeth with implants. So maddening how dentists, orthodontists, and even the modern soft food diet (see the book Breath) has messed our jaws up.

Do you ever hear of complications? Because I see comments under videos on other channels about how painful MSE can be - and even some that it caused permanent chronic pain, permanent neck instability issues etc. Also - does expansion not create a huge wide nose? And how does it open up the actual nasal tubes deep down which is where things get difficult?
You said that it's been life-changing - how so? Also, once the separation occurs, I assume the device can be removed after a certain period of time? Or does some kind of retainer always need to be worn on some kind of basis?
I wish you didn’t have to do so much cutting and surgery for this. My wisdom teeth are soooo close to being able to come in, and they are poking out with no pain. I used to get really congested and now it’s not so bad. I wish I knew about this as a child. I’ve been chewing tough foods and gum, but I know it won’t really be able to do too much to the bones.
Just imagine is MSE could expand over the max of 15 mm. I would go for 25 mm. I also wouldn’t mind it being that wide because it would give me wider cheekbones

It doesn't and he acknowledges this in many videos. But combine MSE with mewing. Once you've already gotten the MSE treatment done, mew with a focus on forward force rather than upward force with the tongue (it's much easier anyways)
Could splitting the maxillary suture have any negative side effects? Would it pose any kind of danger if one tried to split it using the force of their thumbs(similarly to MSE) without pushing on the teeth?
Kiss Ármin Some people have done that lol, I think it may risk failing to expand upper part of maxilla + assymetric expansion. One MSE expansion failed because one screw was not drilled far into the bone and failed to expand upper part of maxilla, the titan screw turned out to be very bent. Dr Won Moon showed it in his presentation.
If u expand slowly there should not be any problems. Check online shop from germany-braces shop. They make removable expander and same results as here described. And u will save so much money. I am so happy with my results. I paid for both mandible and maxilla and shipping 400€
even if you somehow managed to split the suture through face pulling (LOL) you will never expand because you can't hold it in place until new bone grows.
So once it's done youre permanently left with a spilt open gap at the bone-level ?? Generally speaking doesnt that mean less structural integrity if you were to consider the skull or at least the lower half as a single piece?

You didn’t see a diastema because you had a lot of teeth probably. Most of us got badly effected by extractions so we are gonna get gaps. Me for example, I only have 24 teeth and I’m 18

Hello I got 2 premolars extractions on the Upper and 2 premolars extractions on the bottom teeth due to orthodontics. If I expand my maxillary with that device can I replace my extracted premolars?

I don't know the answer to either of these questions. Worst case scenario is that the teeth can be levelled to give the appearance that the cant has been fixed. Thanks for the love.
You can manually try adjusting it, chewing gum to rotate both the maxilla and mandible, and stretching the muscles on the side with the cant. You can look at what parts need upswing, I've noticed chewing gum on front teeth and molars upswing the tip of the nose. I've done this since 2015 and I don't know whether on X-ray I actually improved the cant. So maybe like Ronald said I might've shifted my dental bones instead. But I can tell you my physical appearance improved by a lot, also athletically I feel stronger and balanced, and I learned to sing over these years but noticed the more expansion the more my ribs expand on one side and the more I chew on one side causes my slumped shoulder to be set back permanently. It's crazy how you control your muscles with your face, also while I jog I maintain the weak side of my face semi-contracted as much as I can. This rewires the spine in my experience it expands me even faster and improved voice power. Also my gait cycle became super steady and am getting hunter eyes. Hopefully it is real skeletal change. I wanna see more people do this, it seems like to most it is hopeless. It is hard and I still have a slight cant but the improvement has been great.
@@John-mv1sp if your cant was as significant as mine causing facial asymmetry I might give your method a go, all I really care about now is fixing my asymmetry and maybe getting a perfectly upswing maxilla.

yeah with younger patiens
10-15 mm using biobloc= 5-7.5 mm using mse. Biobloc expands 50% in the dentoalveolar region and 50% along the suture.

Different people have analyzed my photos and seem to believe that Yes, my interpupillary distance has increased. I cannot confirm.

I just found out that they do MARPE in Nijmegen. Might not be exactly the same, but MSE is a type of MARPE.

Is the MSE annoying? Does it hurt the tongue? Does it affect of daily life?
Do you ever hear of complications? Because I see comments under videos on other channels about how painful MSE can be - and even some that it caused permanent chronic pain, permanent neck instability issues etc. Also - does expansion not create a huge wide nose? And how does it open up the actual nasal tubes deep down which is where things get difficult?
So... what about that gap inbetween the front teeth??

You said that it's been life-changing - how so? Also, once the separation occurs, I assume the device can be removed after a certain period of time? Or does some kind of retainer always need to be worn on some kind of basis?
From what I've heard you need to keep the screws in for about 6 months for new bone to form, so that the space created remains.
Can this happened with surgery?
I wish you didn’t have to do so much cutting and surgery for this. My wisdom teeth are soooo close to being able to come in, and they are poking out with no pain. I used to get really congested and now it’s not so bad. I wish I knew about this as a child. I’ve been chewing tough foods and gum, but I know it won’t really be able to do too much to the bones.

Just imagine is MSE could expand over the max of 15 mm. I would go for 25 mm. I also wouldn’t mind it being that wide because it would give me wider cheekbones

It doesn't and he acknowledges this in many videos. But combine MSE with mewing. Once you've already gotten the MSE treatment done, mew with a focus on forward force rather than upward force with the tongue (it's much easier anyways)
Could splitting the maxillary suture have any negative side effects? Would it pose any kind of danger if one tried to split it using the force of their thumbs(similarly to MSE) without pushing on the teeth?
Kiss Ármin Some people have done that lol, I think it may risk failing to expand upper part of maxilla + assymetric expansion.
One MSE expansion failed because one screw was not drilled far into the bone and failed to expand upper part of maxilla, the titan screw turned out to be very bent. Dr Won Moon showed it in his presentation.
If u expand slowly there should not be any problems. Check online shop from germany-braces shop. They make removable expander and same results as here described. And u will save so much money. I am so happy with my results. I paid for both mandible and maxilla and shipping 400€

even if you somehow managed to split the suture through face pulling (LOL) you will never expand because you can't hold it in place until new bone grows.

You should totally do it! I see Dr. Newaz and just got my MSE installed and my suture split and it’s life changing!!! I’m telling everyone about it!!
So once it's done youre permanently left with a spilt open gap at the bone-level ?? Generally speaking doesnt that mean less structural integrity if you were to consider the skull or at least the lower half as a single piece?

Hi Ronald, must a diastema always appear during maxillary expansion? my expansion is almost complete and i dont see a diastema. but my cheekbones are definitely more noticeable. my nose kinda looks the same

You didn’t see a diastema because you had a lot of teeth probably. Most of us got badly effected by extractions so we are gonna get gaps. Me for example, I only have 24 teeth and I’m 18

Despite having a small palate I was able to make mewing a habit. However my tongue touches my molars.. Should i keep doing this and eventually does my palate expand? I'm also considering going for a MSE treatment here in japan in the future.

Hello I got 2 premolars extractions on the Upper and 2 premolars extractions on the bottom teeth due to orthodontics. If I expand my maxillary with that device can I replace my extracted premolars?

I think it doesn't. Skin is much more flexible and just stretches and prob some tissue might grow to compensate the stretch.

Can you talk about maxillary canting? How it occurs and how it can be corrected, love you bro
I don't know the answer to either of these questions. Worst case scenario is that the teeth can be levelled to give the appearance that the cant has been fixed. Thanks for the love.
You can manually try adjusting it, chewing gum to rotate both the maxilla and mandible, and stretching the muscles on the side with the cant.
You can look at what parts need upswing, I've noticed chewing gum on front teeth and molars upswing the tip of the nose.
I've done this since 2015 and I don't know whether on X-ray I actually improved the cant.
So maybe like Ronald said I might've shifted my dental bones instead. But I can tell you my physical appearance improved by a lot, also athletically I feel stronger and balanced, and I learned to sing over these years but noticed the more expansion the more my ribs expand on one side and the more I chew on one side causes my slumped shoulder to be set back permanently. It's crazy how you control your muscles with your face, also while I jog I maintain the weak side of my face semi-contracted as much as I can. This rewires the spine in my experience it expands me even faster and improved voice power.
Also my gait cycle became super steady and am getting hunter eyes. Hopefully it is real skeletal change.
I wanna see more people do this, it seems like to most it is hopeless. It is hard and I still have a slight cant but the improvement has been great.
@@John-mv1sp if your cant was as significant as mine causing facial asymmetry I might give your method a go, all I really care about now is fixing my asymmetry and maybe getting a perfectly upswing maxilla.
